This form is to be completed by new members at Enfield Heights Academy/Kingfisher Hall Academy/Woodpecker Hall Academy School Club only. Please contact your instructor if you are a new member at our outside hall venues (Enfield/Chingford/Collier Row/Chigwell Dojo) as a different form is required.
